Miley's Loving Touch Onsite Diagnostics LLC ("MLT," "we") provides mobile specimen collection and testing in Jacksonville, Florida and surrounding areas. This policy explains what information we collect through this website, how we use it, and the choices you have.

What we collect

• Booking and contact details: your name, phone, email, the address for your visit, company name for group bookings, the services you request, your preferred time, how you plan to pay, and any notes you add.
• Observed drug test collections: the donor's sex, because federal DOT rules require a same-gender observer.
• Partner inquiries: your organization, name, role, email, phone, and message.
• Feedback: anything you share in our survey after a visit.

We don't ask for your medical history through this website, and our forms ask you not to include it. We never collect card numbers on this website. Payments are handled by our payment processor.

How we use it

• To schedule, confirm, and reschedule your visit
• To calculate travel distance and your price
• To contact you about your appointment, by email and, if you allow it, by text
• To send invoices and receipts
• To answer questions and partnership requests
• To improve our service using your feedback
• To keep business, tax, and legal records
• To protect our website from spam and abuse

Text messages

If you check the box allowing texts, we may text you about your appointment, such as confirmations, reminders, and schedule changes. Message frequency varies. Message and data rates may apply. Reply STOP to opt out at any time, or HELP for help. You don't have to agree to texts to book.

We do not share your mobile number or your consent to receive texts with third parties or affiliates for marketing purposes.

Who we share it with

We don't sell your information. We share only what's needed to serve you:

• Service providers that run our business tools, such as Google (email, calendar, maps, and records), Square (payments), Cloudflare (spam protection), and our text message provider
• Laboratories and, for drug tests, medical review officers that process your specimen
• The employer, provider, or organization that ordered your test, as part of that testing process
• Anyone we're required to share with by law

Test results and health information

Specimens and results are handled by the laboratories and medical review officers that process them, under the rules that apply to them. Results go to whoever ordered the test: you, your healthcare provider, or your employer, as those rules require.

How long we keep it

We keep your information as long as we need it to provide our services and to meet business, tax, and legal record-keeping requirements. After that, we delete it.

Security

We take reasonable steps to protect your information, but no website or email system is completely secure. Please don't send health information through our website forms or by email.

Cookies and website data

This site doesn't use advertising or tracking cookies. Our spam protection service and the fonts and icons this site loads from other services may receive basic technical information, like your IP address, as part of delivering the page.

Children

This website is meant for adults. If your child needs a visit, a parent or guardian should book it. We don't knowingly collect information online from children under 13.

Your choices

You can ask to see, correct, or delete the information we have about you by calling or emailing us. We may need to keep some records when the law requires it. To stop texts, reply STOP at any time.
